Care transition coaching.   As seniors move from home to hospital, hospital to nursing home, and back, our coach is there on the phone, helping members and their home caregivers stay on track to avoid re-hospitalization.

 

Medication management.   Many seniors take 6-12 prescriptions as well as over-the-counter medicines.  Senior Dialog^SM tracks these medications using pharmacy data.  But even when data are unavailable or weak, our coaches are specially trained to help members identify lapses and help them develop systems to promote compliance.  This helps our members review their total medication picture with their primary physician.

Surgical decision support.  Many seniors will face a major surgical decision – be it a hip replacement, back surgery, or cataract repair.  With Senior Dialog^SM, they aren’t alone.  Your senior members – and their caregivers – get help understanding the real risks and benefits of undergoing surgery at an advanced age.

Advance care planning. Guilt and worry leads many families to accept over-treatment for their loved ones.  Physicians are trapped, too – without documented end-of-life preferences they must treat aggressively. Senior Dialog^SM facilitates a senior’s end-of-life decision-making before a member is in crisis.
