Health, Dental, and Vision Insurance
Health Dialog offers comprehensive Health, Prescription, Dental, and Vision plans with low employee contribution rates. Coverage is effective on day one of employment.

Flexible Spending Accounts
Health Dialog offers both a Medical and Dependent Care Reimbursement Flexible Spending Account to all employees working a minimum of 30 hours per week.  The maximum annual employee contribution is $5,000 for each account.

Life & Supplemental Life and AD&D Insurance
Employees receive life and AD&D insurance coverage at a rate of 1 ½ times annual base salary up to a maximum of $500,000 at no charge.  Employees may purchase additional life insurance to supplement employer-paid basic insurance.  In addition, this insurance may also be purchased for a spouse and/or dependent.

Short and Long Term Disability
Health Dialog provides Short and Long Term Disability Insurance through The Hartford.

Adoption Assistance
As part of Health Dialog’s commitment to our staff members and community, an Adoption Assistance Program is available to support staff members who are building their families through adoption.  The intent of this program is to provide financial assistance (up to $5,000 per adoption for qualifying expenses) and time off benefits (up to six weeks of time off at 60% of regular earnings) to offset some of the expenses and time obligations associated with legal adoption.

Healthy Lifestyle Support Program
Through our Health Care plan, employees have access to a Fitness Reward, Weight Loss reimbursement program, and A Healthy Me webtool.  Employees may receive up to $150 in reimbursement per calendar year for qualified health club membership, and weight loss program fees.

Retirement
Traditional 401(k) Plan and Roth 401(k)
Health Dialog offers employees the opportunity to save for retirement through a 401K plan and matches all participating employee contributions at 50%, up to the first 6% of their contributions.  

Time Off
Paid Vacation, Personal Time, and Sick Time
Upon hire, staff members will accrue a generous combination of vacation, personal time, sick time or paid time off, totaling up to 4 ½ weeks per year.

Holidays
Health Dialog recognizes eight company paid holidays per year.  However, as an organization that operates 24/7/365, various departments may be required to provide rotating holiday coverage.

Professional Development
Tuition Assistance Program
Health Dialog is committed to fostering the professional growth and development of all employees.  Pre-approved courses that enable employees to broaden their job-related knowledge and improve the skills required to perform their current job assignments or enhance their potential for growth within the Company may be reimbursed at up to $2,500 per calendar year.  Eligibility for Tuition Reimbursement is the first of the month following completion of six months continuous active employment.
