The Patient Profile drives Health Dialog’s predictive modeling, population segmentation, outreach, and reporting efforts. It’s a comprehensive, longitudinal source of current and historical information about the health of individuals and the system in which they obtain care. The Patient Profile considers more than 1,500 demographic and clinical and health system facts derived from medical, pharmacy, laboratory, and claims data. The Patient Profile can be augmented with data from patient surveys, laboratory values, health risk assessments, and other data sources. Applications of the Patient Profile include disease management, case management, opportunity analysis, and internal and external reporting. By including the results from multiple predictive models, the Patient Profile allows clients to segment populations by risk and match the individual to the appropriate level of outreach. The Patient Profile also identifies patients with specific conditions, co-morbidities, and gaps in care – information that can be used to develop targeted interventions to address cost savings and clinical improvement opportunities.
