Based on an Opportunity Analysis of each plan or employer’s population, we develop a customized outreach program that is designed to target those individuals who are identified as high-risk and impactible. To best channel your program investment, the intensity of our outreach is based on each participant’s risk score and willingness to engage in the healthcare process, so efforts are focused where there is likely to be impact.
