Health Dialog has extensive experience in meeting the needs of Medicare beneficiaries. Today, more than 850,000 Medicare Advantage beneficiaries have access to our services through private health plans¹.  Health Dialog is now applying this experience more directly to government purchasers for major government initiatives:

• Health Dialog is working with the Centers for Medicare & Medicaid Services (CMS), delivering chronic care improvement services for the Medicare Health Support Program in Pennsylvania.
• In New Hampshire, Health Dialog is working with Dartmouth-Hitchcock Clinic as part of the CMS’ first pay-for-performance initiative. 

In addition to Medicare beneficiaries, Health Dialog also supports Medicaid needs and federal employee populations:

• Health Dialog is providing Health Coaching services to Medicaid and uncompensated care patients through the Partners HealthCare System, helping these patients work more effective with physicians to manage chronic conditions and navigate the complex healthcare system.
• Health Dialog is providing Health Coaching services to the Federal Employee Program (FEP) for health plans, including Highmark and Independence Blue Cross.
