Coached patients are typically more informed and better prepared, with realistic expectations regarding their conditions and the care they receive. Because coached individuals understand their own roles in managing their care, physicians can expect more productive and collaborative office visits. In many settings, Health Dialog is able to provide physicians with actionable clinical information, derived from health plan claims and pharmacy data, to help support clinical quality and improved patient outcomes1. For example, we offer the SMARTĀ® Registry, which provides actionable information at the point of care for each patient with one or more chronic conditions. It identifies patients who may be candidates for clinical improvement, highlights patterns of care, and identifies opportunities to enhance evidence-based clinical practice. Working with large health plans, we also provide onsite support services for physician office staff to make use of this information.
