Health Dialog’s HEDIS Improvement service provides resources and support to help maximize quality scores. Available as an add-on to our Chronic Care Management program, our service combines analytics, awareness campaigns, and health coaching to close the gaps that can negatively impact HEDIS scores.
Components of our HEDIS improvement program include:
- Powerful analytics that identify patients with gaps in HEDIS measures. Unlike traditional methodologies, our analytics are also designed to stratify and prioritize the populations most likely to close gaps based on past behavior and receptivity to change.
- Engagement campaigns via phone, email, mail, text, and interactive voice recognition outreach. We tailor our messaging based on each person's demographics, clinical needs, geographic location, and more.
- Outbound telephonic health coaching support focused on education and behavior change. Our experienced nurses personalize their outreach to transfer knowledge and address barriers to care.
- Health content that emphasizes the value and importance of preventive screenings and other quality standards.
